The medical community has always taken great strides to protect the confidentiality of their patients' and clients'
health information. Today, State and Federal laws also attempt to ensure the confidentiality of this sensitive information.
The federal government recently published regulations under HIPAA (Health Insurance Portability and Accountability Act)
designed, among other things, to protect the privacy of your health information. This "privacy rule"
protects health information that is maintained by physicians, hospitals, other health care providers and health plans.
This notice describes how information about you may be used and disclosed and how you can gain access to this information.
- Tree of Life Services may use and disclose protected health information for treatment, payment and healthcare operations.
Examples might include, but are not limited to, health information for purposes of life insurance or sports physicals,
medical records for referral to nursing homes, foster care homes or home health agencies and/or referral to other providers
for treatment. Payment examples include, but are not limited to, insurance companies for claims including coordination
of benefits with other insurers and collection agencies. Healthcare operations include, but are not limited to,
internal quality control and assurance including auditing of records.
- Tree of Life Services is permitted or required to use or disclose protected health information (PHI) without the
individuals' written consent or authorization in certain circumstances. Two examples of such are for public health
requirements or court orders.
- Tree of Life Services will not make any other use or disclosure of a client's protected health information
without the individual's written authorization. Such authorization may be revoked at any time. Revocation must be written.
- Tree of Life Services may at times contact the client to provide appointment reminders or information regarding treatment
alternatives or other health-related benefits and services that may be of interest to the individual client.
- Tree of Life Services will abide by the terms of this notice or the notice currently in effect at the time of the disclosure.
- Tree of Life Services reserves the right to change the terms of its notice and to make new notice provisions effective
for all protected health information that it maintains.
- Tree of Life Services will provide each client with a copy of any revisions of the Notice of Information Practice if
there is a need to use or disclose any protected health information of the client. Copies may also be obtained at any time
at our offices.
- Any person/client may file a complaint to the Practice and to the Secretary of Health and Human Services if they
believe their privacy rights have been violated. To file a complaint with the practice, please contact the Privacy &

- It is the policy of Tree of Life Services that no retaliatory action will be made against any individual who submits or
conveys a complaint of suspected or actual non-compliance of the privacy standards.

Tree of Life Services, Inc. does not share or in any other way use information collecting during your use of
our Website without your permission except as outlined in this policy. We do not sell, exchange or trade
customer information with other vendors, trade organizations, direct marketers, or actually, anyone, unless required by law.
Log Files. Like most standard Web site servers, ours writes log files. Logged information includes internet
protocol (IP) addresses, browser type, internet service provider (ISP), referring/exit pages, platform type, date/time stamp,
and page requests. This information is used to analyze site usage trends, administer the site, monitor site bandwidth usage,

User Tracking Technologies. Tree of Life Services, Inc. does not use cookies, web "beacons" or "bugs"
to track the activities of our users.

information when required by law wherein we have a good-faith belief that such action is necessary to comply with a current
judicial proceeding, a court order or legal process served on our website.
Aggregate Information. Tree of Life Services, Inc. may share aggregated demographic information (not personally identifiable)
with partners and advertisers. This information contains our number of monthly page views or "hits", search strings
resulting in visits to the site, type of browser software favored by our users and the site's most popular pages. This is not
